  • Overview

    JOB SUMMARY:

    Provides appropriate and clinically indicated services to veterans. Maintains clinical records within all federal, state, and city regulations documenting all services provided.

    Responsibilities

    ESSENTIAL TASKS:

     

    1. Completes       clinical   assessments   for       Veterans   referred   from       The Department if Homeless Services (DHS).
    2. Provides direct clinical services including individual, group, and crisis intervention as clinically indicated, including (but not limited to): outreach, assessment, evaluation, treatment planning, group and individual clinical psychoeducation and psychotherapy, skills development, crisis intervention, discharge planning, and referral.
    3. Work closely with other community providers, including (but not limited to): psychopharmacology, case management, probation and parole, emergency services, health services, and Adult Protective Services to insure that clients receive comprehensive, coordinated services and treatment.
    4. Collaborates with an interdisciplinary team. Participate in clinical case conferences and treatment planning meetings.
    5. Documents service provision and maintain clinical records in a timely manner in accordance with ICL policies and guidelines, and state and local governmental entities, and CARF accreditation standards.
    6. Complies with managed care requirements including consulting with care coordinators, obtaining treatment authorization, and providing necessary documentation.
    7. Provides services in accordance with ethics, expertise and standards as described in the ICL policy on employee ethics and as defined by the applicable professional ethics-setting group (NASW, e.g.).
    8. Maintains the highest level of professional competence through attendance and participation in staff development training activities

    9. Attends weekly individual supervision with Director of Social Services.

    10. Participate in program development and refinement.

    11. Performs other duties as required.

     

    ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:

     

    1. Ability to work with clients, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences. 
    2. Ability to develop, evaluate, implement and modify a treatment plan, meeting all deadlines and productivity standards
    3. Ability to communicate effectively with staff, clients, families, and the public.
    4. Ability to prepare accurate and timely documentation, reports and other written material as assigned using computers and computer software assigned (e.g., IMA, Word, Anasazi, etc.) 
    5. Ability to understand and adhere to internal and external laws, rules, and policies
    6. Ability to secure the cooperation of and work effectively with others
    7. Ability to work independently, and to conform to all applicable safety and accountability measures

     

    Qualifications

    MINIMUM QUALIFICATIONS:

    Master’s Degree in Social Work from an accredited institution of higher learning and current, valid license (LMSW or LCSW) issued by the New York State Education Department. Maintenance of licensure/certification is a requirement to maintain employment in this position.
