  • Overview

    Under the general supervision of the Health Home Supervisor, responsible for ensuring compliance with all Department of Health and lead Health Homes reporting policies and procedures.


    • Conduct a review of all RHIO & MCO alerts to ensure that, first the alert was forward to staff who will conduct an intervention within 24 hours and document it within 5 calendar days. 
    • Review critical event management to ensure that we are in compliance with DOH & lead Health Homes reporting policies and procedures. Also conduct review Critical event report.
    • Conduct Bi-Annual internal audits of the Health Home Serving Children program.
    • Conduct a chart review and look for interventions applied during and following the event. Also ensure that care plans are modified related to the event whenever necessary.
    • Conduct Training sessions (Documentation, Time Management; Care Coordination protocols, etc.)
    • Conduct the necessary reviews and clear tracking errors for Non-Medicaid related tracking errors.
    • Review Charts and specifically comprehensive assessments and case notes to ensure they are comprehensively & correctly completed.
    • Support SIBN with internal audits intended to address regulatory compliance related concerns.
    • Respond to all compliance audits conducted by the 5 lead Health Homes and work with supervisors to address deficiencies.
    • Conduct internal audits of the 3 care coordination specialty programs (AH+, AOT, & Non-Medicaid) to ensure regulatory compliance.
    • Work with the compliance officers from the 5 lead Health Home to ensure the program is up-to-date with the evolution of policies & procedures.
    • Attend monthly quality management meetings for the 5 lead Health Homes when they are held.
    • Perform other related duties that may be assigned


    • Master’s degree in psychology, social work or related human service field and
    • At least three years’ experience in Health Home care management and Quality Assurance activities or closely related experience
