    This is an entry-level direct care position assigned to community residences. Incumbents are assigned to shifts to insure the provision of 24-hour-a-day, seven-day-a-week coverage.  Workers in this job category perform a wide variety of tasks related to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing, safety, security, and support of children and adolescents with serious emotional disturbance and mental illness, and their families.  These tasks focus on supporting, instructing, and assisting recipients of services to develop the skills needed to attain personal goals, return home whenever possible, live, be educated, work, and socialize successfully in the community environments of their choice and also maintain a safe, clean environment.  In addition, workers also advocate for clients and support families or other caregivers in their efforts to assist these individuals.



    1. Provides individualized care and supervision of each resident.  
    2. Throughout assigned shift performs regular inspections inside of the facility to insure the safety of the residents, accountability of property, cleanliness of residents’ rooms and common areas, cleanliness and security of the area immediately outside the residence, and the security of any assigned vehicles. Insures that only authorized individuals are permitted access to the residence.
    3. Models positive behaviors and implements the core values.
    4. Uses strength-based intervention, communication, and practices to build assets/resiliency.
    5. Assesses, instructs, and measures daily and independent living skills.
    6. Provides a therapeutic milieu and promotes appropriate socialization.
    7. Creates a warm, caring, home-like environment by modeling responsibility and pride in performing household duties. 
    8. Assists and instructs individuals in attending to personal hygiene, grooming, nutrition, and daily living; insures that sufficient and appropriate attire is available.
    9. Participates in the service planning and review process.
    10. Explains the type of services, recreational activities, and ICL programs available to clients; instructs the clients in daily living skills, socialization skill enhancement, and conflict resolution
    11. Delivers services in accordance with service goals and objectives.
    12. Employs trauma-sensitive practices. 
    13. Provides crisis intervention and behavior management.
    14. Accompanies clients on regularly scheduled or emergency visits to medical treatment facilities, social agencies, government offices, or other locations associated with the treatment or assistance of the resident. Accompanies, supervises, and provides transportation (when necessary) for appointments, home visits, sibling visits, and activities.
    15. Promotes family involvement and reunification as appropriate.
    16. Provides life space counseling and support (or provides informal, one-on-one counseling and support).
    17. Ensures adherence to medication regimen. May provide monitoring of medications and make appropriate medication administration form entries in accordance with the medication protocol. May assist residents who are on a monitored medication regime.  
    18. Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Completes incident reports in accordance with ICL policy. Assists the clinical staff in accounting for residents. May assist in the filing of missing person reports on residents not accounted for in accordance with ICL policy and procedure.
    19. Complete all assigned documentation in a timely fashion.
    20. Reviews the staff communication log at the beginning of the work shift and makes entries in the staff communication log concerning security checks, crisis incidents, emergency situations, incident reports, or other essential elements of information designated by program management. 
    21. Executes emergency plans as outlined in the policy and procedure manual under direction of the assigned supervisor and adheres to program policy on securing the safety and well-being of residents, staff, and visitors requiring emergency medical care. Administers CPR or first aid to staff or clients who are injured, as appropriate. 
    22. Observes the physical environment and performs housekeeping tasks as necessary to insure the maintenance of a safe, clean, comfortable, and healing environment for all individuals
    23. Insures that the food provided or selected is appropriate for those on a special diet 
    24. Expected to comply with attendance rules and to report to work as scheduled and/or required on a regular basis. Must have availability and ability to work beyond the normal schedule as needed 
    25. Expected to attend regularly scheduled team and staff meetings and supervision. Complies with all required in-service training and staff development activities and training.
    26. Complies with agency infection control policies.
    27. Assists in residence household duties (general cleaning, laundry, shopping, meal planning and preparation); trains and supervises residents in developing these skills. May assist and supervise residents in meal preparation, laundry, and housekeeping tasks 
    28. Observes individuals’ behavior and specific responses to treatment and rehabilitation programs, support services, and medication and reports observations orally and/or in writing, including computer entries, as instructed and in accordance with applicable audit and agency standards 
    29. May lead skills development or activity groups with clients and families as appropriate 
    30. Other job related duties that may be assigned.


    1. Ability to work with clients/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences. 
    2. Ability to serve as a role model to residents/clients.
    3. Ability to read and write at least at the secondary school level and to follow written and oral instructions.
    4. Ability to react appropriately in emergency situations. 
    5. Ability to complete written forms and reports in an accurate and timely manner, manually and by computer. 
    6. Ability to communicate effectively with staff, clients/residents, families, and the public. 
    7. Ability to accompany or transport residents.
    8. Ability to secure and maintain certification for Standard First Aid and CPR.


    1. Basic knowledge of mental illness and serious emotional disturbances of children and adolescents. 
    2. Basic knowledge of treatment, rehabilitation, and community support programs as they relate to clients/residents, families, and staff.
    3. Basic knowledge of routine clinical procedures and medications.
    4. Basic knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques.

    RELATIONSHIP WITH OTHERS:This position interacts with counselors and case managers and reports to a program supervisor.




    High School graduate or GED plus one year of related human services experience. Specialized training or education in human services may be substituted for experience.  NYS driver’s license may be required of some assignments
