  • Overview

    The Director will play a leading role in the conception, design, and piloting of integrated clinical initiatives to reduce costs to the health system and improve outcomes for the patient and clients served by AsOne’s network of providers. This role will also design and implement provider trainings and assist in value based contracting with payers. This is a unique opportunity to be a key contributor to the development and success of newly formed IPA whose mission is to transform health care through integrated, evidence based, family focused care.

    Responsibilities

    • Lead and oversee AsOne’s clinical strategy through development of standardized integrated care practices, protocols and initiatives with corresponding business and value propositions.
    • Lead and oversee behavioral health-related value based payment contracted projects, working closely with Executive Director and AsOne committee staff, through all phases from initiation/planning through implementation, execution and measurement, ensuring successful outcomes.
    • Collaborate with Executive Director to develop value based reimbursement contracts and sustainability plans for strategic goals and prioritized initiatives, including working with managed care plans and other payers.
    • Provide support in meeting the NYS Behavioral Care Collaborative (BHCC) VBP Readiness grant requirements and deliverables.
    • Develop and implement AsOne network trainings and presentations to support interdisciplinary collaboration, clinical integration and use of evidence based interventions among providers. This includes oversight of training consultants and vendors.
    • Educate network clinical staff on the rationale of protocols, processes, and clinical decision-making criteria of new initiatives and integrated care models of treatment.
    • Support the work of the IT/Data Analytics and Quality staff to ensure AsOne members have information systems, policies and procedures in place to track and share client information and care needs across interdisciplinary team of providers and to monitor client outcomes, identify gaps in care and perform population health monitoring.
    • Work closely and effectively with AsOne Quality staff to capture spirit of clinical protocols in metric identification, prioritization and monitoring.
