  • Overview

    The Nurse Care Manager is responsible for the day to day oversight of the participants receiving medical/psychiatric services at the PROS program. Leads care management services and play a major role in the implementation of integrated health services at PROS.

    Responsibilities

    Nursing oversight of the PROS program, including ongoing nursing assessments, as well as the implementation and evaluation of the following initiatives aimed at improving the mental health and physical health of participants in the PROS program.

     

    • Clinical staff training in disease management
    • Integrated health programming for participants
    • Medical equipment for the PROS program
    • Nursing best practices
    • Coordination between providers, pharmacies, participants and their caregivers

     

    Oversight of the above requires:

     

    • Coordination with the PROS staff, and supervisors
    • Participation in provision of nursing care
    • Collaboration with the Psychiatrist, Primary care Physician, Social Worker and caregivers to ensure safe and effective goal setting and community integration
    • Assistance with interventions related to Smoking Cessation which may be delivered through Intensive Rehabilitation Interventions
    • Administration of IM medications
    • Screening, assessment and follow up
    • Education of participants in individual and group settings
    • Administrative duties as needed, including:
      •   PROS staff and clinical meetings
      • Interdisciplinary case conferences
    • Collaboration with Program Design, Evaluation and Systems Implementation department and coordination of the collection of health data.
    • Liaison with community and hospital providers
    • Performance of other job related duties that may be assigned.

    Qualifications

    Education: RN, current NYS License and certification, and eligibility for full and unconditional participation in the Medicaid and Medicare programs are both required for hire and are conditions for maintaining employment in this position title.

     

    Training and Experience: At least one year post RN licensure clinical nursing experience. Job Knowledge: Expertise in all aspects of nursing work with particular expertise in assessment and treatment for clients with serious mental illness. Familiar with primary care best practices.
